Responsibilities:
- Fulfill the clinical communication role between the Company, healthcare providers, and members/patients of clients of the Company;
- Complete comprehensive medical history and assessments of patients with chronic disease, determine needs and the need and implement chronic care management interventions as guided by pre-established MedAssurant protocols and procedures, which includes conducting face-to-face clinical assessments of patients, educate and coach patients in the area of Diabetes including Asthma, COPD, CAD, Heart Failure and/or Hypertension and facilitate necessary referrals to appropriate healthcare providers;

- Coordinate with all resources and personnel necessary to ultimately achieve superior capability, quality, efficiency, success, and financial profitability of the Company’s Risk Adjustment product and service lines;
- Participate with Company leadership in the strategic development of initiatives to identify product and system enhancements which may improve client appeal, process flow, and overall Company business function, industry reputation, and financial performance
Job Requirements: - Bachelor’s degree required; MBA or Master’s degree in a related field preferred.
- Recent senior management experience within Medicare Advantage arena
- Demonstrated leadership ability and success managing a product/account management function and staff. Ability to build and to grow a management function.
- A positive, flexible and self-motivated attitude, with excellent teamwork skills in a multi-disciplinary environment.
- Strong work ethic and top-notch interpersonal and project management skills.
- Professional written and verbal communication skills.
