MedAssurant firmly believes that in-market resources, which enable face-to-face interactions, assessments, and interventions with patients, have a highly important role in positively affecting the quality of care, lowering costs, and achieving the desired outcomes of a disease management and chronic care program. With greater than 1,000 clinical personnel across the country, MedAssurant brings a powerful team to this cause.  Directed by disease identification, stratification, and prioritization interventions; and in concert with communication center based efforts; in-market operations can indeed play an instrumental role in effective chronic care management and monitoring.  Active management efforts facilitate timely, cost effective, and high quality care provision.  Frequent contact with patients and providers also serve to improve the accuracy of clinical documentation, and therefore associated risk scores and related reimbursement accuracy.

While believing in the value of dedicated, high-touch in-market clinical personnel as part of disease management programs, MedAssurant also realizes that different markets and different health plans will have varying needs. Furthermore, in-market solutions may not be a feasible or efficient investment for some plan types. For these reasons, MedAssurant has designed CCS Advantageä to specifically enable a number of differing modular approaches to maximize the benefits of a well-implemented disease management or chronic care solution.

Options:

  • Option I: Provision of Full In-Market Personnel
    For clients desiring a complete outsourcing of the CCS Advantageä disease management and chronic care solution, MedAssurant provides complete staffing of personnel to deliver the maximal impact of program benefit. The healthcare team generally includes field nurses, behavioral specialists, collaborating physicians, social workers, and case managers.  This interdisciplinary team is embedded in the local provider and social services community and has the ability to best communicate with local resources, coordinate care more effectively, and influence member behavior, participation, and outcomes – the ultimate goal of the program.
  • Option II: Support of Client’s In-Market Personnel
    MedAssurant can also provide support tools for the in-market resources made available by client health plans within their local markets. In these cases, MedAssurant is focused on providing the embedded, in-market interdisciplinary team with the training, communication, data collection, informational, clinical protocol and care support tools necessary to enhance the performance of the CCS Advantage™ program.  From role-specific, multi-day training sessions, to a web-based portal that supports face-to-face member information retrieval and collection, MedAssurant’s CCS Advantageä capabilities can be delivered in such a way as to empower a client’s already existing in-market resources.
  • Option III: Remote Support When In-Market Personnel Models Are Not Feasible
    In instances such as Fee-For-Service (FFS) health plan products, regions of diffuse member distribution, or newly established markets initially lacking high member population density, MedAssurant’s CCS Advantageä solution can be uniquely delivered without heavy, localized in-market personnel and associated expenses. In these instances where health plans are dedicated to the benefits of disease management and chronic care solutions, yet have business models or member distributions that make in-market personnel staffing impractical, CCS Advantageä enables a capability to realize significant benefits and material ROI that are unique in the industry.
