Coordinated Communication
Communication is at the core of a disease management program. Known clinical data must be effectively conveyed to those seeking to impact a patient’s care. Care plans and education must be successfully articulated to patients. Appointments or follow-up processes must be coordinated and communicated to patients and their care providers alike. The organized flow of information is critical to the success and efficiency of a disease management program.

MedAssurant’s Clinical Communication Center has multiple industry-leading capabilities that differentiate it within the disease management and chronic care marketplace. A proprietary task management system monitors communications, appointment schedules, laboratory orders, pharmacy prescriptions, changes in care patterns, and inpatient admissions or acute care facility visits. Guided by the intelligent patient disease identification, stratification, assessment, and prioritization garnered in the ISAP™ module processes, the CCS Advantage™ Clinical Communication Center then utilizes the MedAssurant task management system to reliably undertake both administrative and clinical communications to aid the patient, practitioner, and health plan in improving the appropriate disease management. From follow-up for significant medical events, to delivering highly specific educational counseling, the personnel of MedAssurant’s in-house Clinical Communication Center leverage a powerful technological infrastructure to deliver superior reliability and comprehensiveness in the many communications needed to deliver meaningful success in a disease management initiative.

Another component within the MedAssurant Clinical Communication Center capability is the Electronic Patient Assessment Solution Suite, or ePASS™. This suite of web-enabled patient assessment tools enables both MedAssurant’s in-house Clinical Communication Center personnel, as well as practitioners seeing health plan members, to implement sophisticated health risk assessments (HRAs). These HRAs, known as the Smart HRA™ system, have built-in intelligence to take into consideration not only the known medical history of a patient, but also conditions and comorbidities identified as likely or possible by MedAssurant’s risk adjustment analytics. Powered by MedAssurant’s advanced medical informatics engines, the Smart HRA™ drills down on the critical disease conditions, suspected comorbidities, documentation gaps, and evidences of changes in acuity that are uniquely specific to a unique patient. The Smart HRA™ provides impressive efficiency and specificity to gain valuable information on each specific patient, aid the practitioner with actionable information, and improve the accuracy of HCC documentation and associated patient risk scores.

From enrollment outreach and critical laboratory test follow-up, to obtaining patient and provider feedback and delivering best-practices education updates, MedAssurant’s Clinical Communications Center personnel provide an example of the level of superior quality and results that the combination of advanced technological infrastructure working with care-minded staff can deliver.
